THE PROCESS
When your purchase is complete, you will receive a questionnaire to fill out including your emergency contacts, preference times for calling, your time zone, and any pertinent medical information you choose to share.
We will call you within 48 hours after receiving the information to introduce ourselves.
We will review your information with you on the first phone call.
We will make any changes necessary per your request.

While we cannot promise you will receive the same caller 365 days a year, we strive to be consistent with 1-2 “regular” callers that will truly get to know you.

We don’t believe in “automated” calls. Anyone can do that; we believe in the human voice and hearing and understanding what’s going on.
Yes, it’s a short call but we want to know you are truly doing ok every day. You may need more than a few minutes on the phone and we are here to listen.

Case Management- Every 30 days we will review new medications, update and coordinate medical cares if necessary, medical appointment review and wellness goals. We will also facilitate referrals to community, state and federal programs you may not be aware of.
